The role of the coordinator will run and manage all aspects of the In home support & SIL homes they are allocated to. Focused on person centered care, ability to supervise & support all areas of the house and staff working in the houses.
Responsibilities will include (but are not limited to) the following:
* Liaise with support staff ensuring accurate record keep and documentation including progress notes / incidents/behavioural-charts and daily care charts are filled and are uploaded on brevity/share point and managing message box
* Organising monthly staff meetings, Training sign off sheets/acknowledgement forms and conducting house and client risk assessments to identify and mitigate potential hazards.
* coordinating safety assessments, including fire training, and maintaining accurate safety data sheets and chemical registers to ensure compliance with safety regulationsPBSP and RP implementation
* Compliance implementation on ground as per NDIS and safeguard commission guidelines.
* Assess resident's needs related to health, welfare, employment, housing, training, and other appropriate services for the wellbeing.
* Provide disability support with a focus on encouraging independence and managing challenging behaviour.
* Liaise with participants, their families, and multidisciplinary teams to coordinate services and ensure optimal care.
* Accommodate changes in participants' needs, making necessary adjustments in care.
* Engage in regular training to stay up-to-date with relevant disability service guidelines and practices.
* Read previous day client case notes and uploading documents such as signing off on incident reports and liaising with stakeholders around this, both authorised and authorised RP.
* Ensuring house compliance folders are up to date
* Quarterly Participant paperwork review(care plans, risk assessments, etc.)
* Collecting relevant data and writing end of plan reports
* Stakeholder meetings and reporting issues to management
* Training and guidance for new support workers
* Perform other reasonable duties as directed by the company/management from time to time.
