Job Summary
Lead and develop daily programs for seniors in a community setting.
* Coordinate activities that promote independence, connection, and wellbeing for participants in aged and disability services.
* Plan and deliver engaging centre-based activities that cater to diverse needs and interests.
* Manage a team of support workers to ensure seamless program delivery and excellent customer service.
* Maintain accurate records and contribute to quality improvement initiatives.
* Liaise with healthcare professionals, clients, and carers to provide comprehensive support.
Key Responsibilities
1. Develop and implement day programs that align with CHSP, HCP, and NDIS guidelines.
2. Lead, support, and roster leisure and lifestyle support workers.
3. Promote client independence and meaningful social engagement.
4. Maintain accurate client records and ensure quality documentation.
5. Liaise with case managers and contribute to care planning.
6. Support service development, evaluation, and improvement.
Required Skills and Qualifications
* Completed AQF Certificate III relevant to Aged Care.
* Strong communication, coordination, and problem-solving skills.
* Experience supporting aged or disabled persons in health/community settings.
* Ability to lead a team and deliver inclusive, engaging programs.
* Knowledge of Aged Care Standards, CHSP, HCP, and NDIS.
* Confident with Microsoft Office and administrative processes.
