About The Role
This Enrolled Nurse role involves working within a multidisciplinary Transitional Aged Care Program (TACP) team, providing case management under the supervision of a Registered Nurse.
The Aged Care Division delivers a range of clinical and non-clinical services across the Western NSW Local Health District to support older people and their families to remain supported in their homes.
The teams and programs focus on advocacy, restorative care, and reablement to promote wellness and independence. The role offers support within a strong leadership team and presents challenges in a changing Aged Care environment influenced by growing patient needs and health reforms.
