Responsibilities:
* Connecting with clients, families, and health professionals
* Coordinating services and developing individualised care plans
* Supporting smooth onboarding and ongoing engagement
* Making every interaction matter
Requirements:
We're seeking someone who is both caring and capable, with a high level of professionalism.
* Experience in care coordination, case management, or a similar role
* A background in aged care, disability support, or community services
* Excellent communication, time management, and problem-solving skills
* A strong ethical approach and attention to detail
* Confidence using technology and care management systems
* A current driver's licence and access to a reliable vehicle
Qualifications:
* Diploma qualified nurse highly encouraged to apply, RN / EN
* Certificate IV or Diploma in Aged Care, Disability, Community Services or similar (preferred) with minimum 2 years' experience
* Knowledge of Home Care Packages (HCP) and or NDIS (Desirable)
