About the roleThe Central Adelaide Local Health Network (CALHN) Mental Health Clinical Program is made up of a range of multidisciplinary teams providing recovery‐focused clinical services to consumers. These teams include inpatient and sub‐acute, community and rehabilitation teams in youth, adult and older persons' services.
The Social Worker provides consumer focused, strengths‐based, individually tailored clinical rehabilitation services and group programs to consumers of the Mental Health Service. Rehabilitation assists people to acquire and use their strengths, skills, and resources to support successful and satisfying living, learning and working in the environments of their choice.
The focus is on strengths, self‐determination, cooperative partnerships, hope, community participation and citizenship, family and social networks and inclusion, holistic care, cultural diversity and reflection and learning. Services are time limited and provided by working individually, in groups or by linking with non‐government organisations (NGOs) and other community services.
The incumbent works as part of a multidisciplinary team and is accountable for the provision of discipline‐specific services including assessments, planning, interventions and programs to enhance recovery‐oriented outcomes for consumers and their families and carers. The role provides rehabilitation programs to a group of consumers whose severity of mental disorder and/or level of disability necessitates regular review and follow‐up to facilitate improved social functioning and community tenure.
The Social Worker will provide clinical expertise, leadership and support to multidisciplinary team.
