What You'll Be Doing
* The Clinical Lead – Aged Care Services will provide clinical oversight and leadership in the integration, redesign, and implementation of aged care service models across FWLHD, focusing on improving clinical care pathways, enhancing workforce engagement, and optimising service coordination to improve hospital avoidance strategies, patient flow, and community-based care models.
* The Clinical Lead will work collaboratively with aged care service teams, Project Management Office, hospital clinicians, primary health providers, and community-based services to ensure that models of care align with best‐practice principles and NSW Health strategic priorities.

Qualifications
* Registered Nurse with AHPRA registration, with relevant Post‐graduate qualifications relevant to the specialty and at least 3 years full‐time equivalent clinical experience in the specialty of the specified post‐graduate qualification.
* Demonstrated expertise in aged care service models, transitional care, and hospital avoidance strategies.
* Experience in clinical leadership, including developing and implementing models of care, service redesign, and workforce engagement strategies.
* Proven ability to collaborate with multidisciplinary teams, including hospital‐based, primary care, and aged care service providers.
* Strong knowledge of NSW Health aged care policies, funding structures, and compliance requirements.
* Strong analytical and problem‐solving abilities, including the ability to evaluate service effectiveness and clinical outcomes.
* Excellent communication, negotiation, and leadership skills, with the ability to influence stakeholders and drive change.
* NSW Driver's Licence and ability and willingness to travel across FWLHD, including remote and rural locations, as required.
