Position Title:Social Support Worker – Parental BackfillLocation:Panuku (Darwin)Reporting to:Social Support Coordinator/Panuku ManagerPOSITION SUMMARYWDNWPT Aboriginal Corporation EA 2022 – Community Services Workers Level 2 – part-time, 26.6 hours per week. The Social Support Worker acts as a malpa (helper) for dialysis patients and their families through three core functions:KEY RESPONSIBILITIESAct as a Social Support Worker for dialysis patients and their families.Provide practical support for clients with complex needs as directed by the Social Support Coordinator.Work collaboratively with the Social Support, Elder Care, and Clinical Services teams.Assist clients with:Personal shopping and banking.Attending appointments and meetings (medical, housing, Centrelink, legal).Visiting family members and participating in social and cultural activities.Prepare and cook meals for social activities.Organise and participate in WDNWPT-approved social activities.Facilitate personal grooming and care.Transport clients to various activities or appointments.Support clients in advocating for their health, housing, financial, family, and cultural needs.Help clients access and negotiate the service sector.Work alongside the Clinical Coordinator to support primary health care needs.Perform other relevant duties within the scope of training and capacity.ABOUT YOUFlexible, collaborative, and solutions-focused support worker with strong stakeholder engagement skills and a passion for health equity.Experienced in working independently across complex, remote and cross-cultural environments with empathy, cultural sensitivity and professionalism.Committed to improving outcomes for Aboriginal communities by providing practical, respectful, and person-centred care.Adept at building trusted relationships, navigating service systems, and supporting clients to advocate for their health, social, and cultural needs.SELECTION CRITERIASkills and/or experience in Social Support Services.Open driver’s licence and willingness to travel to remote communities.Current National Criminal History Check (within 6 months).Up-to-date COVID-19 vaccinations (2 doses plus booster).Willingness to obtain Food Safety Certificate, First Aid and CPR if not already held.DesirableAbility to facilitate group activities or cultural programmes that promote social connection and wellbeing.Basic knowledge of food handling and nutrition, especially for clients with chronic health conditions.Commitment to ongoing professional development and a willingness to attend training relevant to the role and community needs.WHAT WE OFFERAttractive salary with 11.5% super + generous salary packaging options6 weeks annual leave plus 17.5% leave loadingOther generous leave entitlementsWe offer relocation assistant to support your move to this beautiful part of Australia
