Salary: $78,000 – $80,000 per annum plus superannuation
Dignified Home Care is seeking a qualified and experienced Residential Care Officer to provide person-centred support services to individuals living with disability. The role involves assessing client needs, developing and implementing individual support programs, and coordinating services to enhance client independence and wellbeing in home and community settings.
Key Responsibilities
• Assess client needs and develop individualised care and support plans
• Implement and monitor support programs in accordance with NDIS goals
• Evaluate client progress and maintain detailed case notes and reports
• Conduct ongoing risk assessments and identify changes in client needs
• Liaise with families, allied health professionals and external service providers
• Coordinate access to community services, health supports and resources
• Participate in case reviews and contribute to continuous improvement of support strategies
• Provide guidance and support to promote independent living and community participation
• Ensure compliance with NDIS standards and organisational policies
Skills and Qualifications
• Diploma or higher qualification in Community Services, Disability, Social Work or related field
• At least 1–2 years relevant experience in disability or community care services
• Demonstrated understanding of NDIS framework and person-centred practice
• Strong assessment, case management and reporting skills
• Excellent communication and stakeholder engagement abilities
