Support Coordinator Role
Our organisation is seeking a Support Coordinator to deliver individualised support to National Disability Insurance Scheme (NDIS) participants. The successful candidate will work collaboratively with service providers and other stakeholders to achieve participant goals.
Key Responsibilities:
* Provide individualised support to NDIS participants, ensuring their needs are met and exceeded.
* Collaborate with service providers to develop and implement person-centred plans in consultation with participants and their families.
* Ensure accurate record-keeping and reporting in accordance with regulatory requirements, maintaining confidentiality at all times.
Requirements:
* Experience working in the community or disability services sector, with a proven track record of delivering high-quality support.
* Strong organisational skills and attention to detail, with the ability to work independently and as part of a small team.
* Effective communication and interpersonal skills, with the ability to build strong relationships with participants, families and colleagues.
