Job Description

The Opportunity

As a Family Liaison Officer, you will provide support to adolescents (aged 10 – 14) and their families who are enrolled at Nambour State College and are disengaged, or at risk of disengaging from school. This role supports the family with parenting skills, wellbeing support, student school engagement and connecting families to additional services where necessary. The support has been established as a targeted response through a needs assessment and includes case management, practical in-home support and strategies for parenting adolescents.

Key Responsibilities

* Complete the primary operational tasks and manage the client's journey through the service with a client centered approach.
* Receive referrals, undertake assessments of client's needs, coordinate case management and regularly review case plans through liaison and collaboration with the Program Coordinator.
* Provide information, advocacy and support using a collaborative case plan model to at risk children and their families in accordance with the requirements of the Service Agreement with the Department of Communities.
* Develop and present individual and group life skills and personal development programs for children and their families who have complex needs.
* Provide additional support to clients as required including transport to school, medical appointments and informal counselling.
* Develop a comprehensive network of relevant agencies, both Government and non‐Government, to support the needs of clients through collaborative case management support.
* Engage in networking and relationship building, undertaking partnerships with schools, government and non‐government organisations, as well as other organisational services to ensure the best outcomes and services for clients.
* Refer client families to appropriate external providers/services as required to support case management and client outcomes.
* Provide practical support to clients including engaging in safety planning, parenting skills and life skills workshops and budgeting or other areas as required to provide holistic support.
* Liaise with external providers including mental health, legal, law enforcement and Centrelink services and personnel.
* Explore / provide opportunities for education / social / personal / development and recreational activities with client families.
* Ensure culturally competent practice when working with Aboriginal and Torres Strait Islander families.
* Record all client data, case notes, reports, according to Youturn policies and procedures and funding body requirements, in a professional manner and within required time frames.
* Represent Youturn Ltd and participate in relevant forums, meetings and networks related to the provision of support services for families and children.
* Maintain key program deliverables as outlined in individual work plans and job descriptions and in line with the Human Services Quality Framework standards.

Desired Skills And Experience

Essential Experience, Qualifications & Other Requirements

* Minimum qualification of relevant Diploma in the Human services fields i.e. Mental health, Family and Youth intervention, Counselling, Family therapy, Community services.
* Minimum 3 years of extensive experience working with children, young people and families with complex needs.
* Experience supporting clients facing significant and varying complex challenges, with knowledge of relevant theory and legislation, supported by a practice approach that aligns with the Strengthening Families, Protecting Children framework and broad-based safety and risk assessment skills including the application of a Safe & Together model of practice.
