About the Role:
Position:
Enrolled Nurse Care Coordinator
Location:
Bundall, Gold Coast, QLD (Onsite)
Hours:
Full-time, Monday to Friday, 8:30 AM – 4:30 PM
Reporting to:
Care Manager
Travel:
Covering Gold Coast North & South regions
Key Responsibilities:
Conduct client assessments and develop personalised care plans.
Coordinate high-quality community care services and monitor client progress.
Provide clinical guidance and support to care staff.
Maintain compliance, accurate documentation, and care coordination.
Work collaboratively with clients, families, and healthcare professionals.
What We're Looking For:
AHPRA-registered Enrolled Nurse
with a passion for community care.
Experience in aged care or community nursing (preferred).
Strong knowledge of My Aged Care and Home Care Packages.
Excellent communication, problem-solving, and organisational skills.
Ability to work both independently and as part of a team.
