Job Title: Educational Case Manager Specialist
We are seeking an experienced and dedicated Educational Case Manager Specialist to join our team.
In this role, you will work collaboratively with a multi-disciplinary team to deliver intensive case management, provide transition supports, and manage education, intervention, and prevention initiatives related to students who are at risk of disengagement or already disengaged from education.
Responsibilities:
* Deliver Intensive Case Management: Work collaboratively in a multi-disciplinary team to deliver intensive case management and provide transition supports.
* Manage Education Initiatives: Manage education, intervention, and prevention initiatives related to students who are at risk of disengagement or already disengaged from education.
* Establish Partnerships: Establish and sustain effective partnerships with stakeholders, government organizations, and non-government organizations.
* Facilitate Transition Plans: Facilitate and lead the coordination of individual student plans and transition plans using an integrated approach that considers a student's strengths and presenting concerns.
Required Skills and Qualifications:
* Understanding Disability: Knowledge and understanding of the impact of disability on individual functioning within an education context and evidence-based practices to support reasonable adjustments.
* Cultural Competence: Experience working with First Nations communities, families, and students from culturally diverse backgrounds.
* Youth Support Needs: Experience working with young people who demonstrate a range of support needs.
