Applications are sought from suitably qualified Registered Nurses to work within the Community Rehabilitation Program as the Clinical Care Coordinator at Echuca Regional Health. This rehabilitation role is currently a max term contract, however the possibility for ongoing opportunities in the future does exist.
Key responsibilities include:
Deliver high-quality, client-centered rehabilitation programs, ensuring safe and effective treatment and rehabilitation planning
Develop and implement creative self-management strategies to support clients in achieving their rehabilitation goals
Manage client data, tracking key performance indicators, maintaining waiting lists and ensuring accurate reporting of client outcomes
Foster a collaborative, positive workplace culture by establishing effective interpersonal relationships and contributing to team-based goals
Actively participate in mandatory meetings as required and requested
Qualifications/experience required:
Current AHPRA registration as Registered Nurse
Minimum of 5 years' post graduate experience
Experience in case management support
