Support Worker
We are excited to offer multiple positions for both Permanent and Part Time roles as Support Workers throughout our supported independent living and inclusion services.
* This role offers rotating roster options, allowing for a flexible work schedule that can accommodate various personal commitments and lifestyles.
You will provide exceptional person-centered support to our customers and empower them to achieve their identified goals and improve their well-being.
Key Responsibilities:
1. Collaborate with customers to develop individualised support plans that incorporate their identified well-being needs and goals.
2. Provide support to customers to improve their independence by assisting with living skills, transport, social activities, medication, and personal care.
3. Offer psychosocial support to customers to explore their practical and complex needs.
4. Keep customer records up to date daily using online systems.
5. Develop and maintain professional relationships and networks with customers, families, caregivers, and key stakeholders.
6. Seek out innovative ways to enhance the customer journey.
Skills and Qualifications:
Punctuality is essential in this role. We also look for individuals who are friendly, outgoing, non-judgmental, flexible, and possess lived experience.
* We require technical literacy with proven ability to use Microsoft Office and reporting systems.
* Certification in Mental Health or Disability is preferred, but equivalent industry experience is also considered.
* Availability to work across a rotating roster is necessary.
* Willingness to use personal mobile phones for work purposes is required.
* A valid NSW Drivers C-class license is necessary, along with the ability to drive large vehicles.
* A willingness to obtain an Australian Federal Police Check, WWCC, and NDIS WC is also required.
