Eunoia Lane currently provides Support Coordination, Specialist Support Coordination, Case Management, and Occupational Therapy services to clients living with disability or mental health diagnoses.

We are seeking a proactive Social Worker/Specialist Support Coordinator to join our enthusiastic team.
The Role
This position involves working with clients with complex care and medical needs across the lifespan, to identify and address barriers to the implementation of their NDIS plan. The role requires building relationships with clients, their families, disability service providers, clinical staff, and government departments including health, education, communities, and justice.
Eunoia Lane promotes a collaborative approach among stakeholders to ensure supports are delivered in a person-centred and timely manner. The role involves establishing a foundation for best practice care, staff training, and supporting clients through crisis to achieve positive and sustainable outcomes.
Key Requirements
* A bachelor’s degree in social work or relevant discipline (Occupational Therapy, Physiotherapy, Psychology, Nursing, or other Allied Health)
* Strong oral, written, and interpersonal communication skills
* Ability to work independently and as part of a team
* Active listening skills and empathy
* Organisational skills with effective time management and task prioritisation
* Working with Children card, NDIS Worker Screening, Police check (within 6 months), current Drivers Licence, and a roadworthy vehicle
* Proficiency in Microsoft Office
* Experience working with clients with dual diagnoses; experience in Forensic Disability or Forensic Mental Health is desirable but not essential
* Willingness and ability to travel by car, 4WD, or light aircraft to rural and remote settings
* Experience working with Aboriginal and Torres Strait Islander peoples and an interest in indigenous advancement
* Experience working with CALD communities and support networks
