About The Opportunity
Primary and Community Health Social Work Department is seeking an experienced Senior Social Worker to support vulnerable families with children aged 0-5 in the Campbelltown and Camden LGA’s through psychosocial assessment, support and intervention.
The role supports the Sustaining NSW Families Child and Family Health Nursing team at Rosemeadow Community Health Centre.
Sustaining NSW Families works with families identified as needing additional support based on the Support Families Early – Secure Start assessment guidelines. The role of the Senior Social Worker within this nursing team is diverse as it works with various internal and external stakeholders to provide service collaboration, case management and review.
The Senior Social Worker is responsible for providing psychosocial interventions and support from a strengths-based model, providing holistic care within trauma informed, culturally safe interventions. Social Work ensures psychosocial supports and interventions are established in collaboration with the family and are adaptive to reflect their changing needs.
In addition, the Senior Social Worker supports other Social Work clinicians, Social Work students and leads Quality Improvement Projects across the wider Primary and Community Health Social Work Department.
What You'll Be Doing
This position is responsible for providing specialist Social Work service to consumers and their carers to ensure the provision of optimal health outcomes consistent with SWSLHD policies and procedures, standards, relevant legislation, and the NSW Health Code of Conduct. The position will also participate in professional development, clinical supervision, teaching, quality activities and other relevant departmental and administrative duties.

With over 800 dedicated staff, including Aboriginal Health Workers, nurses, doctors, and allied health professionals, the service ensures high-quality care close to where people live.
