Job Title: Advanced Practice Nurse Specialist
About The Role
This service addresses a gap for high-risk patients requiring specialist intervention to avoid unplanned admissions. The Advanced Practice Nurse Specialist provides clinical leadership within the Preventative Integrated Care Service, utilising contemporary evidence-based practice to guide safe and quality care across West Moreton Health.
The APC is accountable for clinical triaging and referral coordination, working collaboratively with Nurses and Senior Medical Officers to ensure safe delivery of nursing care to chronic condition patients.
About You
* Contemporary clinical knowledge in sub-acute and chronic disease management, collaborating with individuals and peers to achieve improved health outcomes.
* Leadership in coordinating nursing services across the continuum of care, delivering competent nursing care in complex situations.
* Able to adopt digital technology and utilise digital care solutions as part of delivering care closer to home.
* Advanced level delivery of interpersonal and communication skills, demonstrating autonomous and critical thinking capabilities.
Mandatory Requirements
* Vaccinated against certain vaccine preventable diseases.
* Proof of qualification and registration with the Australian Health Practitioner Regulation Agency required.
