ATSICHS Brisbane is a not-for-profit community owned health and human services organization delivering on the unique health and wellbeing needs of Aboriginal and Torres Strait Islander people in greater Brisbane and Logan. We are the largest, most comprehensive Aboriginal Medical Health Service in Queensland, and Australia's second oldest. Our services include medical and dental clinics, mums and bubs programs, an aged care facility, family and child safety services, social and emotional wellbeing services, kindergarten programs and a youth service.

Our vision for the future is to deliver healthy, connected Indigenous urban communities. To do this we are focusing on five strategic priorities:

Champion healthy individuals and thriving families

The Family Support Worker will provide social and emotional support, and health education to women and family members throughout the woman's pregnancy and up to three years following the birth.

The key functions of this role are to build culturally safe relationships with the families, provide education, offer support and facilitate appropriate referrals.

Key tasks of the BiOC Family Support Worker include:

* Identifying families who require additional support and provide internal/external referrals
* Develop and action in-depth holistic family case plans to address support needs
* Supporting families to gain access to educational programs specific to the family's needs
* Deliver evidence-based and culturally appropriate clinical, health promotion, and educational services to clients and their families
* Support and advocate for the appropriate clinical care of mothers, babies, and the wider family group throughout pregnancy, post-natal care, and through early child development (including 715 Health Checks).
* Work in collaboration with the wider team to best support the families
* Assist with transporting clients under the direction of the Program Manager
* Assist and collaborate with the Administration team to ensure the smooth and efficient running of the Hub as directed by the Program Manager where required
* Any other reasonable tasks as directed by the Program Manager

Desired Skills and Experience

To perform this role, it is essential that the person is an Aboriginal or Torres Strait Islander person. It is therefore a genuine occupational requirement under section 25 of the Anti-Discrimination Act 1991.

Demonstrated experience in facilitating positive behavior change with individuals, families, and community groups.

* Minimum Diploma level qualification in Human Services, Community Services, Psychology, Social Work, or related field
* Experience in case management
* Experience working within the health sector
* Experience working one on one with a medical professional to achieve program outcomes
