Our Community Support Team Leader plays a major role in operations to ensure Empower Healthcare's utilization targets and Service Level Agreements are being met within the Client Services Team through referrals, scheduling and amazing customer service.

**The Role**
Duties will include, but are not limited to;
- Lead scheduling and coordination of Community Support Services within your state.
- Collaborate with the National Operations Manager and Clinical Team Leaders to develop services and teams.
- Plan, review, and develop Community Support Coordinators to meet service agreements.
- Support service improvement projects and contribute to organisational growth.
- Optimize staff deployment and resource allocation to meet national priorities.
- Oversee rostering, workload distribution, and performance management.
- Ensure compliance with policies, health, and safety regulations.
- Track team KPIs and set targets aligned with business goals.
- Handle reporting, process efficiency assessments, and complaint resolution.

**Preferred**:
The Skills and Qualifications required for a Community support Team Leader are:

- Strong team leadership with a lead from the front approach.
- Outcomes focused and ability to solve problems and execute plans.
- Strong customer service skills and phone etiquette
- Proficient computer skills
- Stake holder management and relationship building
- Enthusiastic and Proactive attitude
- Flexible and ability to work with conflicting deadlines and tasks.
- Excellent time management capabilities
- Excellent verbal and written communication skills
- Ability to work within a team and be a team player.
- Ability to work autonomously.
- Adaptable and ability to multitask.
- Exceptional attention to detail

**Desirable**
- Experience, ideally in a Home Care or NDIS setting
- Knowledge of healthcare and medical terminology including in Home Care Package/Assistance, NDIS, Support at Home
