The primary aim is to provide generic care coordination in ED and the community to facilitate self-management strategies, improve health outcomes, and prevent avoidable hospital presentations.As an Occupational Therapist, your responsibilities include providing assessment, intervention, and management to support functional recovery, encourage self-management, and promote empowerment.
