Care Partners play a pivotal role in the care journey of our customers.
This position involves guiding customers through their care path with empathy, working closely with families, clinical teams, and external providers to ensure safe, personalized care.
The Care Partner will be responsible for developing comprehensive health plans, coordinating services, and ensuring that customers receive seamless care.
Key responsibilities include building strong relationships with customers, conducting onboarding assessments, and arranging multidisciplinary services. Collaborating with clinical teams, families, and external providers is crucial to delivering optimal outcomes.
Maintaining accurate records, participating in audits, and contributing to quality improvement initiatives are essential aspects of this role.
The ideal candidate will have experience in community or aged care, including case management and coordinating multidisciplinary care in the home. Strong customer focus, excellent verbal and written communication skills, and the ability to handle sensitive conversations with empathy are necessary qualifications.
Requirements
* Relevant qualifications (Certificate IV or Diploma in Community or Aged Care desirable).
* Experience supporting care transitions and knowledge of funding streams, quality standards, and audits.
* Strong understanding of care coordination and ability to work collaboratively as part of a team.
