We are seeking a dedicated and compassionate individual to join our team as a Care Coordinator. This role involves planning and managing care services for clients receiving in-home support. You will work closely with our field staff, clients, families, and health professionals to deliver services that make a positive impact on people's lives.
As a Care Coordinator, your key responsibilities will include:
* Conducting initial assessments and developing personalized care plans.
* Coordinating and scheduling care services with accuracy and efficiency.
* Supporting carers with guidance, resources, and regular check-ins.
* Maintaining compliance with care standards and documentation.
* Ensuring clients feel heard, respected, and supported at all times.
What We're Looking For:
We are looking for someone who is both caring and capable. You will ideally have experience in care coordination, case management, or a similar role. A background in aged care, disability support, or community services would be an advantage. You should possess excellent communication, time management, and problem-solving skills, as well as a strong ethical approach and attention to detail. Confidence using technology and care management systems is also essential. A current driver's license and access to a reliable vehicle are required for this role.

Qualifications:
A Certificate IV or Diploma in Aged Care, Disability, Community Services, or a similar qualification is preferred, with a minimum of 2 years' experience. Knowledge of Home Care Packages (HCP) and/or NDIS is desirable.
