As key contributors to this Program, our Aged Persons Mental Health Services provide a range of community, bed-based and specialist services to patients in the Casey, Cardinia, Greater Dandenong, Kingston and Monash Local Government Areas.
The Aged Persons Mental Health services have seven teams within the program which are based in Kingston Centre, colloquially known as 'Middle South' and in Endeavour Hills. The Residential Support Program (RSP) is one such team that provides assessment and treatment for people aged 65 years and over, who have a psychiatric disability and/or dementia with associated behaviours of concern requiring specialist treatment and who live in a Residential Aged Care Facility or similar supported accommodation. RSP is staffed by a multidisciplinary team of health professionals. Working together, they provide a comprehensive person-centred approach to mental health care. The teams comprise Psychiatrists, Nurses, Social Workers, Occupational Therapists and Neuropsychologists.
**About the Role**
The Team Leader will provide expert knowledge within their position whose clinical judgements will be recognised as authoritative. As a clinical leadership role, the incumbent will draw on knowledge in mental health legislation and policy to achieve service expectations and performance indicators.
All Team Leaders across the Aged Mental Health Service assists the Community Services Manager to provide outstanding and effective leadership, coordination and management of clinical services for the Aged Mental Health Service. The incumbent will have good leadership with a desired understanding of change management processes to implement and support models of care that ensure streamlined and effective access to services, service delivery and coordinated care.
The Team Leader will also undertake small operational responsibilities under the guidance and directive of the Community Service Manager and be ready to 'step up' into acting Community Service Manager if/when required.
The Team Leader is a role model for the core values and purpose of the organisation to ensure achievement of exceptional care and outstanding outcomes. The incumbent supports excellence in interdisciplinary clinical practice and clinical education, and promotes the development of their direct reports.
The incumbent will develop strong relationships with key stakeholders both within Monash Health and externally.
Having a dual role of providing clinical expertise and leadership to the Aged Persons Mental Health Triage team the Team Leader is expected to demonstrate a range of clinical and leadership competencies which are covered in detail in the Position Description.
About You
**What you need**:
- Minimum 5 years experience as a Registered Psychiatric Nurse Grade 3, Grade 2 OT/Social Work preferably in an community mental health service
- Experience working with Older Adults/ Aged Mental Health Service
- Current Driver's Licence and access to a reliable vehicle
- Sound knowledge and understanding of the Mental Health Act, 2014, amendments and other relevant legislation
