Role Responsibilities:
•Coordinate formal and informal supports
•Manage clients with complex needs and handle crisis situations
•Collaborate with a Multidisciplinary Panel
•Engage with Mental Health, Justice System, Housing, and Health sectors
•Undertake clerical administration and report writing
•Network and communicate with key stakeholders
•Work independently
•Work in a sensitive, complex and demanding environment with a high degree of autonomy and self-management.
Specialist Support Coordinator (SSC) specific responsibilities:
* Develop and implement support plans for participants with high-level risks or barriers
* Conduct detailed risk assessments and implement mitigation strategies
* Stabilise crisis situations and coordinate urgent, intensive supports
* Navigate complex service systems including justice, homelessness, and hospital settings
* Collaborate with statutory agencies, guardians, and tribunals in high-risk cases
* Prepare evidence-based reports for complex funding requests
* Participate in NDIA Complex Support Needs Panel reviews and case conferences
Qualifications & Experience:
•Relevant qualification in the Human Service Sector or a minimum of 3 years' experience in a similar disability sector role
•Preferred: Degree in Social Work, Psychology, Occupational Therapy, Counselling, Human Services, or Disability Studies
•Experience in Forensic Disability or Forensic Mental Health is a plus
•Demonstrated in-depth knowledge of the NDIS and experience working with its participants
•Valid Driver's License with a suitable, insured vehicle
•A current Working with Vulnerable People registration
•Recent police check (or willingness to obtain one)
