Role Description
This is a casual role for a Community Support Worker, based in Albany, WA. In this position, you will provide holistic support to individuals in the community through assistance with daily living tasks, coordinating community access, and offering both emotional and practical support to facilitate independent living. You will also work closely with case managers, other support staff, and healthcare teams to ensure a high standard of care is provided to meet the unique needs of each client.
Qualifications
* Strong interpersonal skills and the ability to build rapport with clients
* Proficiency in community support and community health service delivery
* Effective communication skills for collaborating with clients, families, and care teams
* Experience in case management or a related role
* Prior experience working in the disability or healthcare sector is advantageous
* Knowledge of NDIS and the ability to work within its framework is highly desirable
* Empathy, patience, and a person‐centered approach
* Valid driver's licence and access to reliable transportation
