Job Description:
* Supporting older adults to live safely and independently at home by prescribing equipment, coordinating home modifications, and delivering person-centered care in the community.
Required Skills and Qualifications:
* Must hold registration with the Occupational Therapy Board of Australia.
* Proven ability to exercise professional judgement commensurate with years of experience and to establish effectively collaboratively working relationships within a multi-disciplinary team and inter-agency networks.
* Demonstrated well-developed interpersonal, written and verbal communication skills with the ability to communicate with internal and external stakeholders.
* Demonstrated ability to prioritise and manage workload demands, meet deadlines and complete activities (including ability to work within scope of practice and seek assistance when required).
* Demonstrated commitment to participation in professional development, quality improvement activities and workplace education.
