About the Role
We are seeking an experienced and motivated Occupational Therapist to join our dynamic and supportive team, contributing to high‐quality, person‐centred care for older adults across our acute services as the Allied Health Acute Aged Care Coordinator (OT). The role provides expert clinical leadership and consultancy, facilitating integrated assessment, care planning, and timely discharge for eligible aged‐care patients.
Working collaboratively with multidisciplinary teams, you will identify older consumers who would benefit from comprehensive care planning, including behavioural support, mobility strategies and diversional programs. You will act as an NALHN specialist in aged care, dementia, and behaviours of concern, fostering strong working relationships with community partners, non‐government organisations and the Department for Health and Wellbeing.
A key component involves monitoring and escalating system issues affecting patient flow and discharge timelines, supporting inpatients with complex needs through time‐limited case coordination, comprehensive assessment and tailored strategies that promote safe, sustainable discharge and prevent unnecessary hospital representation.
