Our Occupational Therapists and Exercise Physiologists play a vital role in supporting patients through recovery, rehabilitation, and everyday activities.
What You'll Be Doing
The Senior Clinician Community Health will be responsible for providing support to patients with late stage chronic and complex health conditions, including:
* care planning and advance care planning
* palliative care needs assessments and collaborative treatment care plans
* referrals to relevant internal and external health and community support providers
* education/support to other clinicians in supporting complex patient needs
Selection Criteria
* Current Registration as an Occupational Therapist with the Australian Health Practitioners Regulation Agency (AHPRA) or eligibility for accreditation as an Accredited Exercise Physiologist with Exercise and Sports Science Australia (ESSA)
* Demonstrated extensive knowledge and experience working with consumers with chronic and complex illnesses
* Demonstrated experience of care planning and case management of patients in the community with chronic and complex illnesses
* Demonstrated understanding of specialist palliative care, a palliative approach and community health services
* Demonstrated high level of interpersonal and communication skills
* Proven clinical leadership capabilities
* Current unrestricted drivers' licence and the ability to maintain
