Key Job Responsibilities
The Physical Therapist will carry out an individualized program of physical therapy to maximize a patient's progress toward achieving functional goals.
* Creates, implements, and executes patient treatment plans.
* Communicates with patients, families, referring physicians, and other members of the team.
* Provides timely written documentation per facility and department requirements and as required by insurance provider to meet licensing, certification, regulatory, and reimbursement requirements.
* Develops and enhances clinical and professional skills through knowledge and professional associations.
* Supports and participates in department operations and development.
* Reports any changes in patient status or needs to Director of Rehab and interdisciplinary team as appropriate.
Requirements for Success
To be successful as a Registered Physical Therapist (RPT), one must possess the following qualifications:
* Current Connecticut License as a PT (Physical Therapist)
* Current CPR
* At least one (1) experience working in a Skilled Nursing Facility preferred.
