As a Case Manager in our Home Care team, you'll be supporting both our clients and the care workers who assist them day to day.
* Planning and coordinating care services
* Supporting and mentoring Home Care Workers
* Working alongside GPs, allied health and families
* Ensuring services align with best practice and client needs
* Building strong relationships with clients and their circles of care
Responsibilities:

2. Managing care plans and responding to client needs
3. Providing support and guidance to care workers
4. Collaborating with healthcare professionals and families
5. Maintaining strong relationships with clients and their networks
