This organisation is a growing and dynamic not-for-profit, committed to supporting elderly Victorians to continue living in their own home. Having expanded significantly in the last three years, we are looking for a Care Advisor to join the team. Centred on Bayswater, this provider delivers services in the Southern and Eastern suburbs of Melbourne.
**Opportunity**:
Working out of the Bayswater head office, as part of the Home Care Packages Team, the Care Advisor is responsible for providing care plan advice and referrals to up to 50 clients and their families/carers, enabling them to remain at home as long as possible.
Reporting to the Operations Manager, the Care Advisor is responsible for:
- Providing guidance and advice to clients and their families on the range of aged care services available, including government-funded programs, private providers, and community-based services
- Ensuring recommended services and supports are in line with client's funding, care plans and individual goals
- Conducting risk assessments
- Liaising with service providers, allied health services and community organisations, to ensure a coordinated approach to service delivery
- Working collaboratively with the Service Delivery team to arrange referrals as required
- Monitoring and reviewing clients' packages on an ongoing basis, making changes as required to ensure that the client's needs are met and that the care provided is effective
- Maintaining accurate and timely records of client interactions, assessments, and care plans
**About you**:
We are seeking a driven individual who is passionate about supporting clients to achieve their goals:
- Registered or Endorsed Enrolled Nurse
- Flexible and adaptable learner
- Excellent communication and interpersonal skills with the ability to build strong relationships with clients, families, colleagues and service providers
- Understanding of the Aged Care sector, including government-funded programs and community-based services
- The ability to work independently and as part of a team, with a strong focus on client outcomes and satisfaction
- Strong organisational and time-management skills, with the ability to manage a busy workload and multiple deadlines
- Current Victorian Drivers Licence and access to a vehicle with comprehensive insurance (kms reimbursed)
