Job Title: Support Worker
About the Role:

* The successful candidate will be responsible for ensuring the well-being and safety of participants under their care.
* Excellent communication skills are essential for this role, as you will be interacting frequently with patients to identify their needs and attend to them accordingly.
Main Duties:
1. Communicate effectively with participants to understand their needs and provide appropriate support.
2. Ensure appointments are made and kept with physicians as required.
3. Be responsible for the well-being and safety of participants under your care.
4. Interact respectfully with participants and their relatives when necessary.
5. Keep all matters related to participants or their families confidential at all times.
6. Extend support periods if needed.
7. Notify the participant's physician and family in case of serious health-related issues.
8. Assist with household duties such as laundry, cleaning, meal preparation, gardening, shopping, and supporting participants in their interests and hobbies.
9. Assist participants with personal care needs.
Requirements:
* Certificate IV in Disability or equivalent.
* Current driving license.
* Current CPR/First Aid certification.
* Minimum two years' experience in disability, community, or acute care.
