**Monash Health is excited to announce an opportunity for a Full-time Home Care Package Case Manager to join our Community Support Options team; Fixed-term until January 10th 2025 (LSL cover).**

**About The Role**

Community Support Options is part of Monash Health Aged Community. Our services are delivered to carers and care recipients in the municipalities of Casey Cardinia, Greater Dandenong, Kingston & Bayside. Community Support Options offers an exciting opportunity for a suitably qualified, experienced and motivated Case Manager within our Home care package program (HCP) in line with the aged care reforms. The successful applicant will have a proven track record in delivering the flexible, re-enablement and wellness focussed model of care case management and monitoring a HCP caseload of client. Relevant tertiary qualifications in health/social sciences and previous experience within the field will be positively welcomed. You will also need to demonstrate an understanding of aged care reforms and client directed care. Good computer, communication and organisational skills; and the ability to work autonomously and as part of a team are essential.
- You will be an experienced person that can provide comprehensive client assessments and develop care plans to support consumers in their own homes;
- Provide holistic care skills within a case management framework;
- Consult, network and liaise with the rostering staff and relevant agencies, contractors, service providers and key stakeholders;
- Contribute to the ongoing development of the Home Care Program, including participating in development of procedures, planning and reviewing of care recipient services;
- Meet weekly with the Care Manager and Team Leader to provide reports on achievement of KPI's and budgets;
- Liaise with multiple service providers to ensure care recipient needs are met in line with the client care plan;
- Monitor and review ongoing consumer needs, physical and emotional wellbeing and make appropriate recommendation of supports and services;
- Actively respond to new customers, manage referrals, client details and upgrades to consumers on the My Aged Care Services Portal;
- Analyse client issues, problems solve to achieve realistic and practical client centred solutions;
- Understand and prepare consumer budgets;
- above all, be proactive and client focused.
