We are seeking a skilled professional to fill the role of Bed Day Discharge Coordinator at our hospital. This position will involve working with a great team to support quality patient care and deliver exceptional services.
About the Role

As a Bed Day Discharge Coordinator, you will be responsible for providing consultative and liaison-focused services aimed at supporting medical and nursing staff in the assessment, care, management, and successful discharge of patients.
Key Responsibilities
1. To provide a consultative and liaison-focused service aimed at supporting medical and nursing staff in the assessment, care, management, and successful discharge of patients with necessary family and community organisations to ensure the smooth and safe transition out of hospital.
2. To ensure effective and timely admission/discharge processes are in place and to case manage patients with an extended length of stay.
Requirements
To be successful in this role, you will need to have:
* Registered with AHPRA as a Registered Nurse
* Previous experience in community and/or facility (or similar role)
* Effective communication skills at all levels enabling you to achieve best practice
