Clinical In-Home Services:
* Care plan assessment, medication management, wound care, chronic disease management, palliative care, and health management
* Clinical supervision and leadership of non-clinical team members in a community setting
* Client intake processing, clinical care planning, and reporting
* Conducting client assessments with a focus on Consumer Directed Care
* Coordination of quality person-centred care according to client needs, service plans, and available funding
* General administration for documentation related to client care and in alignment with relevant regulations and legislation
Required Skills and Qualifications:
* Qualified Enrolled Nurse with current registration
* First Aid and CPR certification
* Australian National Police check
* Professional Indemnity Insurance
* Experience in assessing and planning care needs within the community
* Valid Driver's Licence and reliable vehicle
