About Lives Lived Well
At Lives Lived Well, we help people impacted by drugs or alcohol as well as mental health. We get lives back on track by delivering a wide range of programs and services (including residential rehab) across 40 locations in Queensland and New South Wales.
About the Role
As a Pre and Post Worker you will work closely with our Shanty Creek residential treatment facility and clients to enable smooth transitions both into and exiting the facility which include services that support the continuing care of clients. This would include doing assessments in community and residential settings.
In this position you will be guided by your Team Leader to provide high quality engagement with clients to admit them to the Shanty Creek facility and to discharge from Shanty Creek facility. You will also be working closely with our intake and assessment worker to plan pre and post support for our clients.
The role will also include:
* Provide recommendation for entry to Shanty Creek based on the results of assessment, screening and risk assessments
* Assist in the coordination of entry for new residents on their first day, confirming or organising Transport support and timely admission with the Shanty Creek team
The successful candidate will need:
* Tertiary qualifications in a relevant human services discipline with additional AOD qualifications highly regarded
* Applied knowledge in contemporary evidence based AOD counselling practice, case management, intake, and assessment
* Demonstrated expertise in residential treatment service delivery with a sound knowledge of the potential risk elements specific to this type of environment.
* Experience in a role where you work over the phone.
* Local area knowledge, especially of community service agencies and programs
* Experience in working in/with an outreach service
* Current Australian Drivers Licence
