Support Coordinator
We are seeking a Support Coordinator to support participants, their family and carers in identifying and connecting with appropriate service providers.
About the Role
* The Support Coordinator is responsible for delivering high-quality support coordination services to assist participants to implement their NDIS plan by maximizing choice and control and engaging high-quality service providers.
* Establish and maintain clear lines of communication and consultation between participants, their guardians, families, and other service providers.
* Ensure participant goals and level of attainment is regularly reviewed, adjusted and recorded.
Key Responsibilities
* Select preferred options or providers based on selection criteria.
* Arrange necessary assessments to determine funding requirements.
* Monitor and adjust participant budgets to optimize NDIS plan outcomes.

Qualifications/License/checks
* Cert IV Qualification in Community Services Sector
* Apply for NDIS Worker Screening Check
* Working with Children Check
* Current Driver's Licence and Vehicle
