Care Companion Role
The role of the Care Companion is pivotal in enabling older individuals to maintain their independence and live safely in their own homes. This is achieved through the delivery of care that is centered on the individual's needs, preferences, and cultural background.
Key responsibilities include:
* Establishing genuine partnerships with customers and their supporters.
* Conducting comprehensive assessments to identify a customer's strengths, needs, goals, preferences, and cultural background.
* Coordinating various services and supports to respect customers' choices and ensure seamless transitions.
* Monitoring customers' wellbeing and adjusting resource allocation accordingly to meet evolving needs.
To excel in this role, you will require:
* A Certificate III in Health Services Assistance or an equivalent qualification.
* Proven experience in providing care management or case management services.
* Exceptional skills in assessment, support planning, and coordination.
