Job Title: Integrated Care Coordinator

This is a great opportunity to work in a multidisciplinary team providing clinical leadership and care coordination support to consumers of the service. The role involves working with people with mental illness, providing expert clinical skills and knowledge to members of the team.
Key Responsibilities:
* Provide clinical leadership within the community mental health team.
* Work collaboratively with a multidisciplinary team to provide care coordination support to consumers of the service.
* Cultivate meaningful relationships with consumers and carers, with a commitment to recovery principles in mental health service provision.
Requirements:
* Registered Nurse (Division 1) with current AHPRA registration and post-registration qualifications in Mental Health, plus at least 3 years' experience working in Mental Health.
* Demonstrated skills in mental health assessments and interventions, as well as experience working with individuals with mental illness.
* Able to provide leadership to a multidisciplinary team across services and settings, including skills in clinical support planning and integrated care coordination.
