About the role
As a Case Manager, you provide holistic support to a caseload of clients by assessing their situations and identifying strengths, challenges, and needs. You develop comprehensive care plans with clear goals and interventions tailored to each client. The role involves implementing these plans by linking clients to appropriate services and advocating on their behalf with relevant stakeholders. You also monitor, evaluate, and document client progress, adjusting care plans as needed to ensure effective support.
About the program
The Cairns Education, Training and Employment Program (CETEP) program provides a range of learning training opportunities and employment pathways for young people living in Cairns and Yarrabah. CETEP engages with young people who are at risk of disconnecting from their family, community or support networks, disengaging from school, training and/or employment, harm and/or homelessness. The aim to ensure that young people; experience positive relationships with their family and community, are engaged in education, training and/or employment, lead healthy and violence free lives and have safe and stable places to live.
To be successful you will need
* Knowledge of stator requirements, the role, organisational programs, policies, and operations.
* Sound discipline knowledge with experience, training, and education.
* Specialist understanding of the principles in the role, such as a degree with relevant experience and / or Associate Diploma with extensive case management experience.
* Lesser formal qualifications may be acceptable. However, substantial years of relevant experience would be required.
* Experience working with Aboriginal and Torres Strait Islander people, families, and communities.
* Experience working with people from culturally and linguistically diverse (CALD) backgrounds.
Compliance
* Evidence of required qualification
* Satisfactory outcome of a current (less than 3 months old) National Criminal History Check
* Blue Card (QLD working with children card)
* QLD drivers’ licence ‘Class C’ or interstate equivalent
* Out of hours may be required from time to time
* Ability (and willingness) to undertake local/regional travel
* First Aid certificate and CPR certificate
