Role Overview
The Case Manager position plays a pivotal role in enhancing community wellbeing by delivering holistic case management services.
Working closely with participants referred through the Home Detention Integrated Support Services Program (HISSP), you will assess and coordinate referrals, providing support across a range of identified needs.
This involves timely assessments and making referrals to internal services as part of the program, as well as external supports if longer-term needs are identified.
Key Responsibilities:
* Deliver comprehensive case management services to programme participants
* Coordinate referrals and provide support tailored to individual needs
* Collaborate with disadvantaged individuals interfacing with the criminal justice system
Requirements:
* Hold tertiary qualifications in Social Work or a related human-services discipline
* Demonstrate proven case management experience, preferably in working with ex-offenders
* Exhibit effective time management skills while working autonomously
