**Full-time or Part-time role, based in Ballarat**
**About the role**:
We are seeking an experienced Support Coordinator, to join our dedicated team in Ballarat. In the role, you will strengthen participant's ability to connect with, and maintain supports outlined within their NDIS plan. You will also be developing professional relationships with participants, families, and other service providers.
**Key Duties and Responsibilities**:
Applicants **must** have experience in NDIS Support Coordination or NDIS Planning. Service Delivery:
- Support Coordination
- Plan implementation
- Progress and monitoring
- Review and Reporting
Applicants should have a minimum of 12 months experience in Support Coordination or NDIS Planning and have a sound knowledge of the NDIS and local services.
