Job Title: Rehabilitation Specialist
We are seeking a skilled Rehabilitation Specialist to join our team. This part-time role involves conducting assessments, identifying individual capabilities and preferences, and setting realistic goals that help maintain or restore independence for all of our customers.
Key Responsibilities:
* Conduct resident assessments, determine individual capabilities/preferences, and set achievable goals
* Support residents in achieving these goals while maximizing their functional capacity for daily activities
* Prescribe aids and equipment as needed
* Maintain effective communication with residents, their families, and the multidisciplinary team
* Supervise and support therapy assistants
* Address the concerns and requests of residents' families and other caregivers
* Collaborate with the multidisciplinary team to ensure comprehensive care
* Identify areas for improvement and use a continuous quality approach to problem-solving
Requirements:
* Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Rehabilitation Specialist (essential)
* Previous experience in rehabilitation (desirable)
* Proven ability to assess, plan, implement, and evaluate quality care
* Empathy for elderly clients and the ability to work effectively with other health professionals and service providers
* Excellent interpersonal and communication skills
* Current Australian driver's licence
