**Location: Lillydale, VIC**

Kuremara provides a wide range of services, programs, and support for people with disabilities, we have over 10 years of experience in leading the delivery of primary health care services in Rural Queensland. We bring together a diverse set of skills from a wide variety of healthcare backgrounds to provide quality support services for our participants.

**About the role**

We are looking for a **clear English speaking** - **Female SW**who is hard-working and dedicated to join our team. You will need to be friendly, positive and demonstrate client-focused behaviour, showing empathy and treating others with dignity and respect. You'll also need to have a natural ability to build relationships that make a difference, be honest, helpful and dependable, making you a great asset to our team.

As a Disability Support Worker, you will play a crucial role in providing personalised support to individuals with disabilities, with a focus on those with mental health needs.

**Positions available**:

- In-home support / Assistance with Daily Living, Community Access and Transport - (Monday-Friday & Weekends)

**Position Overview**:
As a Support Worker at Kuremara Care, you will play a crucial role in providing support to the client. We are seeking a compassionate and dedicated **Female support worker** to join our team.

**Should be,**
- **Fluent and clear English speaking skills.**:

- Middle-aged, enthusiast, and easy to get along with.
- Someone who takes initiative and can hold a good conversation with the participant
- Experience working with individuals with diverse needs is preferred.
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a team.

**Responsibilities**:

- Provide one-on-one support and assistance to individuals with diverse needs.
- Working within a person-centred, enabling environment that respects the rights and needs of each client
- Assist with daily living activities, including personal care, meal preparation, household tasks and domestic duties
- Assisting clients with daily errands, companionship and support
- Providing support with academic, social or recreational activities
- Preparing food, feeding clients and assisting with medications
- Observe, care and report any changes in the health and well-being of the client
- Engage the individual in social and recreational activities, fostering independence and community integration.
- Maintain accurate records and documentation as required.
- Collaborate with a multidisciplinary team to ensure the individual's needs are met effectively.

**Key Responsibilities**:

- Community Participation**: Facilitate and engage in community activities to promote social integration and participation.
**- Personal Care**:Assist with daily living activities, ensuring the comfort and well-being of the clients.
**- Transportation**: Safely transport clients, creating a supportive and secure environment.
**- Appointment Assistance**: Accompany clients to appointments and provide necessary support.

**Requirements**:

- Experience**:Proven experience as a Mental Health Disability Support Worker, with a strong background in understanding PBSP and community participation and personal care.
**- Qualifications**: Relevant qualifications in disability support or a related field.
**- Communication Skills**:Excellent communication and interpersonal skills to build rapport with clients and work effectively within a team.
**- Driver's License**:A valid Australian driver's license is essential for transporting clients.
**- Availability**: Flexible availability, including weekends.

**Essential Criteria**:

- Understanding of safe work practices.
- Previous employment experience in the health/Disability care sector.
- Ability to work independently and accurately report back to supervisors.
- Flexibility to work various shifts, including evenings and weekends.
- Experience in providing quality services that assist people to maintain their independence, dignity, cultural values and religious beliefs.

**Qualification**:

- Support worker: At least 1 year of experience
- NDIS Worker's Orientation
- NDIS Check - Yellow Card
- Working with Children Check (Blue Card)
