The Opportunity
As a Family Wellbeing Service Practitioner at Kambu Health, you will play a vital role in supporting First Nations children, families, and communities to build strength, safety, and self-determination.
Working within a culturally strong, community-led organisation, you will provide case management, therapeutic support, advocacy, and practical assistance to families, ensuring they are empowered to lead their own decision-making and achieve positive, sustainable outcomes.

What You'll Do
* Deliver support and coordination services consistent with the Our Ways Statement
* Undertake comprehensive family assessments in partnership with families
* Develop case plans identifying priorities, actions, and internal/external supports
* Maintain a caseload with regular contact and follow-up to progress case goals
* Work closely with Family Wellbeing Support Workers and relevant team members
* Provide culturally appropriate social, family, and advocacy support, including parenting education
* Support meaningful engagement of children, parents, and family networks in all decision-making processes
About You
· You are empathetic, resilient, and committed to walking alongside families on their journey.
* Strong cultural awareness and respect for First Nations communities
* Experience in case management, family support, social work, or community services (desirable)
* Strong communication skills with a compassionate, professional approach
* Ability to manage competing priorities and maintain accurate documentation
* Commitment to confidentiality and ethical practice
* Ability to work both independently and within a supportive team environment
* Current QLD Driver's Licence
* Blue Card and National Police Check mandatory (ability to obtain)
