Job Purpose:
The SIL (Supported Independent Living) Facilitator performs an integral role in ensuring the efficient running of the SIL House by co-ordinating customer, staff and Just Better Care services.
Main Duties & Principal Accountabilities:
* Provide guidance, education and direction to staff in relation to tasks that require completion during services.
* Provide all supports as required: Personal Care, Social Support, Transport, Assist/Administer Medications, Nursing Cares (as required), Domestic Assistance, overall monitoring their well-being and reporting on progress.
* Oversee general maintenance of house hold facilities.
* Ensure compliance with NDIS and relevant regulations applicable to SIL house.
* Understand customer service needs or preferences, ensuring all relevant information is accessible for team members and service providers.
* Identify and participate in personal professional development.
* Ensure compliance with all Just Better Care policies and procedures, statutory and contractual requirements in relation to management of service delivery.
* Manage communication with key stakeholders, internally and externally.
* Apply and uphold principles of equity and anti-‐discrimination in the workplace and adhere to organisational and legislative Health, Safety and Environment requirements.
* Any other duties as required to meet the ongoing needs of the organisation.
Selection Criteria:
Essential:
* Qualifications in Nursing (Diploma or Bachelors)
* Previous experience supervising, supporting and training staff.
* Experience maintaining computerised customer information systems.
* Administrative and organisational skills.
* Written and verbal communication skills.
* Demonstrated ability to contribute positively to the team.
* Current and clear Criminal Record Check.
* Current Working with Children Check (where required in accordance with state based legislation).
* Current Drivers Licence.
* Current NDIS Workers Screening Clearance.
