About the Transition Care Program
The Transition Care Program is a goal oriented restorative program that provides short term and active management for older people who have been hospitalised and who require more time and support in a non-hospital environment to complete their recovery. It is a versatile care program that offers case management along with low intensity rehabilitation, nursing, personal care, and other support services and may be delivered in a community or residential package of care.
About Your New Role
As a compassionate and skilled Social Worker, you will play a vital role in supporting older patients during their recovery journey at home or within community settings. This position involves conducting home visits, providing tailored support, and working in accordance with professional standards. You will collaborate with patients, families, and multidisciplinary teams to establish community supports, connect individuals with essential services, and identify suitable residential care options for seamless transitions upon program discharge.

Key Responsibilities
Provide efficient and responsive social work service appropriate to a patient group and their families/carers
Undertake community and home visits to our elderly clients
Case management of clients including the coordination of internal and brokered services
Work collaboratively and cooperatively as a member of a multidisciplinary and Social Work team
Qualifications and Experience
Tertiary degree in Social Work
Eligible for membership to the Australian Association of Social Workers (AASW)
Aged Care Check (GCH will cover cost)
Driver Licence
Evidence of vaccination, or be able to be vaccinated against and remain vaccinated against - Measles, Mumps, Rubella, Varicella, Pertussis and Hepatitis B
Tuberculosis screening, to identify if the tuberculosis vaccination may be required
Declaration of serious discipline history must be disclosed

Salary
Temporary full time (up to six months, with the possibility of extension)
Base salary ranging from $3030.00 - $4453.50 per fortnight
