Full time and Part Time PPT minimum 3 days (22.5 hours) opportunities
The yiriyirimbang Case Manager plays a vital role in providing Permanency Support Planning (PSP) for Aboriginal children and young people living in Out of Home Care (OOHC), their families and carers.
The Case Manager will be responsible for providing all aspects of case management support for Aboriginal young people, their families and foster/kinship carers.
You will also need to work in collaboration with families and community stakeholders to assess and work towards permanency for each young person. This will include helping them to build connections in the community as well as assisting them to independent living.
To be successful in this role you will need to demonstrate the following:
* Relevant Diploma inwelfare, community services or related field with experience in working with children and families
* Ability to work rostered on-call shifts and travel requirements
* Demonstrated adept knowledge, skills and experience working with Aboriginal children, young people in the OOHC/PSP setting
* Practical experience of working as a multi-disciplinary team
* Advanced level of oral and written communication skills, including record keeping
* Knowledge of and Commitment to privacy and confidentiality
* Commitment to participate and complete all mandatory training and formal qualifications
* Possession of a current 'C' Class Open Driver's Licence valid in NSW is required

Remuneration: dependent on skills and quals
