Your new role: In this role, you will provide advanced diabetes nursing care to clients with complex needs across community and home-based settings. Working autonomously within a multidisciplinary team, you will deliver assessment, care planning, education and coordination while contributing to clinical leadership, quality improvement and service development. You will: * Deliver specialist diabetes assessment, care planning and clinical intervention * Coordinate and support complex chronic disease management in the community * Provide education and support to patients, families and carers * Apply advanced clinical judgement in autonomous practice * Contribute to quality improvement, education and service initiatives About you: You are an experienced Registered Nurse with strong clinical capability in diabetes care and a passion for community-based chronic disease management. You will bring: * Current NMBA registration as a Registered Nurse * Advanced experience or interest in diabetes and chronic disease nursing * Strong clinical reasoning, education and care coordination skills * Ability to work autonomously within multidisciplinary teams * Commitment to evidence-based practice and patient-centred care Check out our quick tips and application essentials!
