 
        
        The purpose of the Clinical Nurse Consultant HEaLS (Helping Enhance all Long stay and Subacute patients) role is to provide an enhanced patient journey and discharge planning via a central point of coordination to work towards broader systemic solutions for sub-acute and long-stay patients with complex clinical needs. 
About the Team
Children's health Queensland Hospital and Health Service offers hospital and health services for children and young people across Queensland and northern New South Wales. We deliver a full range of clinical services, tertiary level specialist care and health promotion programs. We work with healthcare partners and families to make sure the care we offer is safe, accessible, high quality and person-centred. The Queensland Children's Hospital (QCH) provides children's health services for children and young people from birth to 16 years of age and up to 18 years for established paediatric patients.
About the Role 
- Provide professional leadership, advocacy, advice and consultation to support and lead multidisciplinary teams to access appropriate and necessary supports, as a part of supporting the continuum of care from hospital to community and access appropriate discharge plans
- Proactive discharge planning in partnership with the family unit and relevant clinical care team
- Interagency communication and collaboration to expedite discharge into the community
- Provide education, coordination and initiation of Subacute and Non-acute Patient (SNAP) across maintenance and rehabilitation care types
- Provide patient and family centred, holistic care to enable timely discharge of patients, including interhospital back transfer initiation
- Organise and coordinate Multidisciplinary Team Meetings (MDT) and long stay meetings to support discharge
